Cardiac biomarkers are measurements taken from the blood to measure the heart’s functionality. They are used extensively in the diagnosis or treatment of heart disease. Although they’re often referred to in the context of heart disease, other diseases may also lead to an increase in cardiac biomarker level. Some of these other diseases include but are not limited to, atherosclerosis, congenital heart defects, left ventricular hypertrophy, left ventricular dilation or LVH, congenital cardiomyopathy and myocarditis. An increasing number of cardiologists are incorporating new methods in assessing the cardiac function of patients with various diseases.

There are a variety of different cardiac biomarkers available. These diagnostic tests measure specific proteins that are present in the blood (or other biological fluid) and can be detected in a routine blood test measures total count and peak contractility (the ability of the heart to contract). In a recent study by Collins et al. it was found that a significant proportion of patients with congenital heart disease had abnormalities in their coronary enzymes measured via a sleep test called the electrocardiogram (ECG).

This study was looking for evidence of protein accumulation in left ventricular wall fluid, particularly in children with congenital heart disease and in a normal population. Interestingly, the researchers found that the ECG recorded elevations in myoglobin which correlated with poor heart function measured via EKG. The next big discovery was that the protein accumulation in left ventricular wall myoglobin was only seen in people with acute myeloid leukemia. This study showed that the oxidative stress of chemotherapy caused the myoglobin to accumulate and become oxidized, causing a rise in its level as measured using cardiac enzymes. Using cardiac enzymes showed that the myoglobin content in the heart was indeed increasing due to chemotherapy, in agreement with the hypothesis of the researchers.
